  Getting in contact with the office and setting my first appointment with Dr. Hernandez was effortless, and he along with the rest of the staff made an excellent first impression. He reviewed my medical history, listened to my concerns, asked appropriate follow up questions, and by the end of the appointment I walked out with orders for a script, labs, and diagnostics that were relevant to everything that I brought up with him. I intend to have my labwork done at their onsite lab within the week, and I scheduled my follow up appointment for next month easily through their online patient portal.

  I was relieved to see De Perales and establish her as my Primary Physician. She’s smart, caring, and listens carefully. The office is well run too.

  I have been a patient of Dr. Zuzak, both myself as well as my son. I can’t say enough of the friendly professional staff and Dr. Zuzak is very thorough in combination with her bedside manners. If all offices provided this service and medical knowledge in plain English, we’d all be happy all at once. I’m very grateful!

  I just moved here to AZ from PA and needed to find a PCP right away for some health issues I have. I read a good review about Live Well Family Practice so I decided to go to this practice and I am so glad I did! The staff, especially the front desk staff, are very friendly and so helpful! I have had two visits with Dr. Hernandez. He is so caring and he really took his time with me on both of my visits to make sure I got to talk about everything that is of concern to me. I feel very Blessed to have found such a wonderful doctor??!
