  Rachael with CareChoice has been a very reliable resource to assist with locating personal care homes for a family member as well as help navigating the application for Social Security Disability, Medicaid, Medicare, Georgia's CCSP Program, etc. No matter the question Rachael has been patient and provided a response that I could understand. She has been the liaison with the personal care homes, explained how the various programs worked, what action steps I needed to take etc. Rachael makes it easy. You can tell she really cares. I can continue working and let her make the calls and do what she does best. Need help navigating these foreign waters, seek out Rachael and you will be so glad that you did. It is comforting to know when I need help that I can reach out to her for assistance.

  Care Choice is an excellent way for families to find the resources in their area. She always is there any time, day or night to aid them in their time of need. I love working with her. Her professionalism and undaunted spirit to assure a family gets the best care possible.

  Rachel and her team are awesome! She is very knowledgeable and professional and has the ability to make difficult decisions easier with great coaching and kindness. I would recommend her services to anyone looking for assisted living services.

  My mom had been living with my sister and family for over 15 years when her health began rapidly declining. She’s in end stage renal failure and has diabetes, but was managing both quite well. However, she was rushed to the ER and then ICU with ischemic liver failure. Against the odds, she recovered but we determined she was no longer safe at home. If my nephew hadn’t found her and called 911, we would have lost her. It was clear she needed more help and oversight with daily life without intruding on her relative independence (still cognitively quite sharp but physically needed a hand with some things).
  I was put in touch with Rachel and Amy through the case worker at the hospital. They met with me and very clearly discussed options, budget, and worked incredibly hard to find the right fit for mom based on her needs and lifestyle. They held our hand every step of the way: planning, finding a good intermediate rehab facility, understanding any financial aid options, setting up tours with senior living communities, and filing all the necessary paperwork for us. We would have been totally lost without their exceptional care, responsiveness, and attention to detail. I cannot recommend them enough. They made a very difficult experience so so much easier. My family and I are so grateful for them.

  Our family was in need for a quick decision as to what assisted living facility was best for Mom after a release from a rehab facility. Rachael was a rock star in helping filter through all the choices you can consider. She was wonderful in meeting the needs of my mother with the the most appropriate assisted living facilities. We could not have done it without her. She is so professional and upbeat. She took our stress away with her positive attitude and spirit. We are blessed to have been connected with her.
