1. We do not allow potential patients from any country to use family members identification and insurance cards to obtain medical care. That is considered illegal. You must use your own insurance card authorized by your insurance provider and appropriate identification for your visit.

2. Medications (all) are only prescribed to patients with appropriate needs and with appropriate screening as outlined by the State of Illinois.

3. We do not participate in any illegal activities.
