I was referred to Dr. Formanek thru a co worker, and choosing him is the best decision ever!! I have been dealing with plantar fasciitis for years and never knew until it got unbearable. He has helped me so much.. My feet now are healed and I couldn't have found a better dr to assist me... His team is phenomenal and so sweet, everyone greets you with a smile and so helpful..
