(February 5, 2019, 12:56 pm)My mom was there after a stroke and had the most wonderful care. The staff was attentive to her needs and were kind, understanding and went out of their way to care for her. If you need a rehab facility this is the place to go.

(February 5, 2019, 10:36 am)I had to face placing my grandmother in a facility from the hospital. She ultimately went to hospice house instead and passed. However, in the mean time I worked with this facility and they were very thoughtful in this process. I would use them again in the future for my family if it ever came to that again.

(January 18, 2019, 3:56 pm)My mother recently spent 2 months in rehab at Five Oaks. The staff was wonderful. Very attentive to her personal and medical needs. I could not have asked for more. Of course, we were thrilled to get her back home but that would.not have been possible with out.the wonderful nursing staff, therapist, Doctors and all the staff there. The facility is clean and bright. She had a large private room and bath which helped her be more comfortable there. I am so grateful for all the care and love that my mother received at this facility. I would.not hesitate to have a family at Five Oaks in the future. If you need rehab FIVE OAKS is the place in Concord to go!
