“I am writing this review to contradict another person's review suggesting this office is difficult with regard to getting medical forms. I am not certain if they were requesting forwarding of complete patient history or just completion of medical release/current physical type information. If it's the latter, we just had to do this for our tween's trip to a Boy Scout summer camp. We dropped off the paperwork, were just asked to complete a portion of it (general history) and sign the parent/guardian portion and show my driver's license/ID. I was advised the form should be ready to pick up in 3-5 days and they would call to let us know. For privacy reasons, they can't email it but could also fax it to a number we provided if desired.
