The patient forms required are reasonable - there isn't a ton of repetitive info for you to fill out multiple times, which as a patient, I really appreciate. The pre-appointment interview I had was thorough and pleasant, and Dr. Slaughter was very thorough and personable. I'm pleased that I found this provider and will feel comfortable going back.
