The Virginia POST Collaborative is pleased to announce that as of November 2016, the Code of Virginia (12VAC5-66-10) has been amended to include POST! You may have heard about a new kind of health care directive in Virginia, called a Physician Orders for Scope of Treatment (POST) form.
Additionally certified by the Virginia POST Collaborative to teach the POST Facilitation Course (Physician Orders for Scope of Treatment). I have trained over 250 Advance Care Planning Facilitators to explore patients' goals and complete POST forms as part of a health care team.
POST was first piloted in Virginia in Roanoke beginning in 2007.Second POST pilot was in Norfolk/Virginia Beach which began in 2011.Since then POST Form pilots have expanded around the state.POST now has the endorsement from the Medical Society of Virginia POST Collaborative.
